Patient Centered Medical Home (PCMH) is a team-based health care model that transforms how primary care is organized and delivered.  We work together to bring you the best care possible, whether it's scheduling appointments, reminding you of lab work needed for your individual disease management or getting you ready for an appointment with your provider.

Led by your primary care provider, your health care team will work together to communicate clearly, enhance access, and coordinate appointments and services to provide the most efficient health care.  Teamwork is an important component for your care.  

Your primary care provider, along with our clinical and front office staff, will work with you to deliver high-quality, efficient care. We ensure equal access to all of patients regardless of source of payment - we have financial assistance programs available and can also help provide information about obtaining health insurance. 

We want you to feel comfortable and confident with the quality of care you receive from your entire medical team because caring for you is our most important job.

From providing preventative care and health education to treating illness and chronic conditions - our goal is for you to be the healthiest you possible.

Be an Active Partner in Your Care
To deliver the best care, we ask that you....
  • Talk to us about your health, illness and concerns.
  • Learn about all medications that you are taking.
  • Ask for medication refills at your office visit.
  • Take part in your treatment plan and let us know of any concerns that you may have.
  • Talk to us about any health care you receive outside of our office.
  • Call us first with all medical problems unless it is an emergency.
  • Give us feedback so we can improve our services.
