About Our Practice

At Primary Care of Milford, our team is dedicated to providing high quality care for you and the entire family. We coordinate your care, from routine screenings to specialty care, to assure you receive the right care at the right time. 

Led by your primary care provider, we work as a team to coordinate your care, communicate clearly, and deliver personalized care.

We offer a range of services, including:

  • routine, preventative care;
  • same-day sick visits; 
  • care for patients with chronic health conditions; 
  • employment, school, athletic and camp physicals; 
  • coordination with specialists and behavioral health support.
Our state-of-the-art facility gives also you access to on-site physical therapy and rehabilitation services as well as behavioral health.
 
Our practice is part of Southern New Hampshire Health, a clinical affiliate Massachusetts General Hospital.
