Patient Information

Make an Appointment

We’re accepting new patients. To make an appointment:
  • Call (603) 595-0063 during business hours (Monday to Friday, 8:00am to 5pm)
  • Schedule online anytime — please use your full legal name so we can find your medical record. If you do not see a time that works for you, please call our office.
  • To get a same-day appointment, call us as early in the day as possible.
If you need immediate, non-emergency medical attention, please call us. In those instances when we are not able to see you, we may refer you to Immediate Care.

Please arrive 10 to 15 minutes before your appointment.

To transfer your medical records to our practice, print these forms and bring them to your appointment:

After-Hours Care

When our office is closed, you can reach an on-call provider by calling our office at (603) 595-0063.

For life-threatening emergencies, like chest pain or shortness of breath, call 911 or go to the nearest Emergency Department. We also offer walk-in 7-days a week at Immediate Care.

Prescription Refills

You can usually refill your prescription at your visit, or you can use the patient portal, or you can call our office at (603) 595-0489 and leave a message with your prescription information.

Please allow 24 to 48 hours for your refill. Some medicines may take longer because we need to get a prior authorization from your insurance company.

Referrals

Many health plans require a referral from your doctor for other services like specialty care. To get a referral, call our office at (603) 595-0063. Please allow 5 to 7 business days for your referral. Unless you are having a life-threatening emergency, please call our office first before getting medical care elsewhere.

Health Care Forms

If your physical exams and vaccines are current, you can get this information through your patient portal for school, camp or other needs. If you have trouble accessing this information, please contact us, and we will send your forms within 3 to 5 business days.
