Southern NH Health Provider Access Options

Immediate Care:
  • The following Immediate Care locations are open for business as usual for all non-respiratory illness.
    • Amherst
    • Hudson
    • Pelham
    • South Nashua
    • Nashua West Campus
  • Our Immediate Care South Nashua and Nashua West Campus are open for all non-respiratory illness in person as well as telephonic and tele-visual appointments. Appointments are available during regular hours of operations for that location.
  • Our Immediate Care Merrimack location at 696 Daniel Webster Highway in Merrimack, NH remains open for all respiratory visits. Upon arrival, a nurse will meet you at the entrance, complete a pre-screening questionnaire and take your temperature. If you have a fever and/or are experiencing symptoms related to respiratory illness you will be asked for you cell phone number and to return to your car. The nurse will call you to review your symptoms. We will speak with a provider and advise you. If you are experiencing life threatening or emergent symptoms please call 911.  
  • Our Immediate Care Pepperell location at 68 Main Street, Pepperell, MA is temporarily closed. We would invite you to call or visit our other open Immediate Care locations or call your Primary Care Provider for assistance.
 
Primary Care - all locations:
  • Telephonic and tele-visual primary care appointments as well as a limited number of in-person appointments are available for patients by calling their Practice or Primary Care Provider. If your Primary Care Provider (PCP) determines that you should be seen in the office, they will provide you with instructions and schedule an appointment for you. Appointments are available Monday – Friday: regular hours of operations for that practice.
  • If you have questions about an already scheduled appointment, please call your Primary Care Provider/Practice.
  • Walk-in appointments are NOT available at Primary Care Locations. Please see our Immediate Care changes (listed above) for walk-in information at our Immediate Care locations.
 
OBGYN - all locations:


We are committed in continuing to give all of our patients the best care possible.  We are still here for all of your needs during our normal business hours Monday-Friday.  Given current changes in our operations due to the COVID-19 pandemic have made changes in the way we are giving care:
  • We are screening all patients as they arrive within the building and all patient areas are being disinfected several times throughout the day.
  • We are using technology as a bridge to connect with all of our patients via Telehealth or Telemedicine with your team of providers.
  • Appointments for urgent gynecological needs are still available with your current provider team.   
  • Obstetric visits have been transitioned to an in office and a Telehealth/Telemedicine combination care plan.
  • We are still offering all of the same genetic testing and ultrasounds based on patients’ needs.
  • We are practicing and maintain social distancing for our patients, which include reducing the number of patients being seen in the office at a time.  To keep all of our patients and our care team safe we are limiting the number of people in the office at a time. To ensure the safety of our patients and their families we ask that patient attend appointments alone (with the exception of labor and delivery patients).
    • Labor and Delivery: patients are currently allowed to have (1) support person accompany them throughout their stay. That support person must remain on the unit/in the patient's room at all times throughout the patient's stay.

Pediatric Primary Care - all locations:
  • Telephonic and tele-visual primary care appointments as well as a limited number of in-person appointments are available for patients by calling their Practice or Primary Care Provider. If your Primary Care Provider (PCP) determines that you should be seen in the office, they will provide you with instructions and schedule an appointment for you. Surgical & Medical Specialties:

Foundation Medical Partners’ outpatient practices are open for business and are offering alternative ways such as telehealth visits to care for you during this time.  Please call the practice and we would be happy to accommodate your needs.
 
Outpatient Rehabilitation:


SNHH’s Outpatient Rehabilitation Locations will be consolidating due to the Coronavirus Pandemic. Please refer to the updated locations/hours listed below.

Our open locations are only seeing select patients and employees at this time. Should you need to schedule an appointment or have questions regarding an upcoming appointment, please call the appropriate location.

 
Open Locations:

  • Nashua Rehab Clinic, 460 Amherst St, Nashua –Open Monday – Thursday, 7:00am – 5:00pm and Friday, 7:00am – 1:30pm  
  • Nashua Rehab Clinic, 17 Prospect St, Nashua –Open Monday & Wednesday, 7:00am – 4:00pm, Tuesday & Thursday 7:00am – 5:00pm and Friday, 7:00am – 3:00pm
  • Pediatric Rehab Clinic, 5 George St, Hudson –Open only on Monday’s 
 
Closed Locations:
  • Merrimack Rehab Clinic: Effective Wednesday, April 1, 2020 – patients must now utilize Southern NH Rehab Center at 460 Amherst St, Nashua  
  • Hudson Rehab Clinic: Effective Monday, April 6, 2020 – patients must now utilize Bone & Joint Center at 17 Prospect St, Nashua
  • Milford Rehab Clinic: Effective Monday, April 6, 2020 – patients must now utilize Southern NH Rehab Center at 460 Amherst St, Nashua
