Make an Appointment

We’re accepting new patients. To make an appointment:
  • Call (603) 891-0083 during business hours
  • Schedule online anytime with select providers—please use your full legal name so we can find your medical record. If you do not see a time that works for you, please call our office.
  • To get a same-day appointment, call us as early in the day as possible.
Should you require immediate, non-life-threatening medical attention, please call us.  In those instances when we are not able to see you, we may refer you to Immediate Care

Please arrive 10 to 15 minutes before your appointment to fill out paperwork. To transfer your medical records, print these forms and bring them to your appointment:

After-Hours Care

When our office is closed, you can reach an on-call provider 24-7 by calling our office at (603) 891-0083.

For life-threatening emergencies, like chest pain or shortness of breath, call 911 or go to the nearest Emergency Department. Walk-in care is also available 7-days a week for urgent needs at Immediate Care.


Prescription Refills

You can usually refill your prescription at your visit. Or you can use the patient portal or call our office at (603) 891-0083.       

Please allow 24 to 48 hours for your refill. Some medicines may take longer because we need to get a prior authorization from your insurance company.
 

Referrals

Many health plans require a referral from your doctor for other services like specialty care. Please contact your doctor to request a referral prior to your visit with us. If a referral is not on file, we will ask you to sign a financial waiver as a guarantee of payment. 
 

Health Care Forms

If your physical exams and vaccines are current, you can get this information through your patient portal for school, camp or other needs. If you have trouble accessing this information, please contact us, and we will send your forms within 3 to 5 business days.
