Southern New Hampshire Health Recognized for Excellence in Diabetes Education
An estimated 12% of the U.S. population has diabetes, which means that more than 40 million people in the country have the chronic health condition.
The Centers for Disease Control and Prevention says there are several potential factors that have led to an increased number of Americans with diabetes. One reason is that people are now living longer with the condition because of improvements in self-management, lifestyle changes, treatment options, and healthcare services.
The importance of helping people with diabetes learn about their condition and the ways to manage it cannot be overstated. Meaningful
education and support can lead to a longer and healthier life. And it’s why Southern New Hampshire Health once again worked with the American Diabetes Association to be recognized for outstanding Diabetes Self-Management Education and Support.
At Southern New Hampshire Diabetes and Endocrinology, we have a full team of providers working with patients to become more knowledgeable and develop skills to successfully self-manage this chronic health condition. We collaborate and work with patients to solve problems, inspire them to create goals and action plans, and help them understand that life can be extra stressful when living with diabetes.
Part of being officially recognized by the ADA for our education efforts means that patients can be confident Southern New Hampshire Health is here to support them. Our team of nurses and dietitians helps people understand the many ways diabetes affects their bodies, provides guidance and support for lifestyle modifications and keeps them up to date on the newest available medications, and diabetes-related technology.
