Patient Information

Appointment Scheduling

To make an appointment:
  • Call (603) 429-3155 during business hours.
  • Schedule online anytime—please use your full legal name so we can find your medical record. If you do not see a time that works for you, please call our office.
  • To get a same-day appointment, call us as early in the day as possible.
Please bring your insurance card with you, and plan to arrive at least 10 to 15 minutes before your appointment. If you are a new patient, please bring a list of any medicines you’re taking and any allergies you have.

If you need to transfer your medical records, print these forms and bring them to your appointment:

After-Hours Care

When our office is closed, you can reach an on-call provider by calling our office at at (603) 429-3155

For life-threatening emergencies, like chest pain or shortness of breath, call 911 or go to the nearest Emergency Department.

Prescription Refills

You can usually refill your prescription at your visit. Or you can use the patient portal or call our office at (603) 429-3155.

Please allow 24 to 48 hours for your refill. Some medicines may take longer because we need to get a prior authorization from your insurance company.

Many health plans require a referral from your doctor for other services like specialty care. To get a referral, call our office at (603) 429-3155. Please allow 5 to 7 business days for your referral. Unless you are having a life-threatening emergency, please call our office first before getting medical care elsewhere.
