Make an Appointment

To make an appointment:

  • Call (603) 579-9648 during business hours (Monday to Friday, 8:00am to 5pm).

Please arrive 10 to 15 minutes before your appointment. To transfer your medical records, print these forms and bring them to your appointment:

After-Hours Care

For life-threatening emergencies, like chest pain or shortness of breath, call 911 or go to the nearest Emergency Department. We also offer walk-in 7-days a week at Immediate Care.
 

Prescription Refills

You can usually refill your prescription at your visit. Or you can use the patient portal or call our office at (603) 579-9648.       

Please allow 24 to 48 hours for your refill. Some medicines may take longer because we need to get a prior authorization from your insurance company.

Referrals

Many health plans require you to obtain a referral from your primary care provider before seeing a specialist. Please contact your provider’s office to request a referral prior to your visit with us.
