Make an Appointment

To make an appointment:
  • Call (603) 577-5760 during  regular business hours 
  • To get a same-day appointment, call us as early in the day as possible. 
  • To schedule an appointment online, please log into your MyChart account.
  • Are you an established patient at Southern NH Health or The Elliot and not currently enrolled? If so, click here to sign-up and start scheduling online appointments today.
Please arrive 10 to 15 minutes before your appointment. To transfer your medical records, print these forms and bring them to your appointment: Release of Records.
 

After-Hours Care

When our office is closed, you can reach an on-call provider anytime by calling our office at (603) 577-5760.

For life-threatening emergencies, like chest pain or shortness of breath, call 911 or go to the nearest Emergency Department. We also offer walk-in care 7-days a week at Immediate Care.
 

Prescription Refills

You can usually refill your prescription at your visit. Or you can use the patient portal or call our office at (603) 577-5760.       

Please allow 24 to 48 hours for your refill. Some medicines may take longer because we need to get a prior authorization from your insurance company.

 

Referrals

Many health plans require a referral from your doctor for other services like specialty care. Please contact your doctor to request a referral prior to your visit with us.
