Kidney Transplant Evaluation Clinic with Massachusetts General Hospital
Southern New Hampshire Health offers a transplant evaluation clinic in collaboration with Massachusetts General Hospital, increasing access to world-class kidney transplant care closer to home. This program is open by referral from providers with any hospital for their patients. Patients may also directly request an appointment. Evaluations are by appointment only. To make an appointment:

- To get a same-day appointment, call us as early in the day as possible.
Please arrive 10 to 15 minutes before your appointment. To transfer your medical records, print these forms and bring them to your appointment:
When our office is closed, you can reach an on-call provider by calling our office at (603) 577-5377.
For life-threatening emergencies, like chest pain or shortness of breath, call 911 or go to the nearest Emergency Department. We also offer walk-in non-emergency care 7-days a week at Immediate Care.
You can usually refill your prescription at your visit. Or you can use the patient portal or call our office at (603) 577-5377.
Please allow 24 to 48 hours for your refill. Some medicines may take longer because we need to get a prior authorization from your insurance company.
Many health plans require a referral from your doctor for other services like specialty care. Please contact your doctor to request a referral prior to your visit with us.
