SolutionHealth MyChart Patient Portal Now Available for Patients

Southern New Hampshire Health has transitioned from offering the eCare patient portals to SolutionHealth MyChart. This transition is part of a substantial upgrade to our medical records system to Epic.

SolutionHealth MyChart consolidates your medical history into one record available across Southern New Hampshire Health and Elliot Health System. Now, you can receive care at any SolutionHealth location without the hassle of updating or transferring your medical records. Whether you need to see a specialist, visit Urgent Care or anything in between, all of your most important health information is accessible to every medical professional you encounter within SolutionHealth.

As a Southern New Hampshire Health patient, you have been able to use eCare to request appointments and outpatients have been able to complete prescription refills, securely message your provider and review your medical history and lab results. You still have access to these functionalities on SolutionHealth MyChart, but now through a more convenient and user-friendly platform. In fact, you can access SolutionHealth MyChart on your computer, tablet, or smartphone to do any of the following:   

  • Request an appointment

  • Send a message to your provider

  • View a family member’s chart

  • View basic health information

  • View test results

  • Request a prescription refill

  • Pay your medical bills
