About Bonnie E. Duquette, APRN-FNP
Bonnie Duquette became a registered nurse in 2000 and spent 18 years at Southern New Hampshire Medical Center, working across multiple specialties, including medical-surgical, emergency care, surgical services, and administrative roles. In 2018, she earned her nurse practitioner certification at Rivier University and joined Downtown Medical Associates as a primary care provider, where she managed both acute and chronic illnesses in patients with complex comorbidities. There, she emphasized patient education in disease management, risk reduction, and health maintenance.
Bonnie then joined Landmark Health’s home-based medical program, where she cared for a panel of complex adult and geriatric patients, delivering high-quality care focused on preventative medicine and acute management.
Bonnie now spearheads the Interim Home Care program for Foundation Community Care. The goal is to deliver high-risk patients the same quality care they would receive in the office but in the comfort of their own home, helping to keep them out of the hospital, improve outcomes, and support patient engagement. This role provides short-term, supportive care and is designed to address immediate or transitional needs such as post-discharge follow-up, care coordination, and acute concerns.
Interim Home Care does not replace the patient’s primary care provider. Patients are encouraged to maintain their ongoing relationship with their PCP for comprehensive, long-term care. Throughout the delivery of care, Bonnie communicates updates and recommendations to the PCP to ensure continuity.
