Southern NH Health Welcomes Three New Providers

Southern NH Health is proud to welcome three new providers to the system. Dr. Le, Rachel Demers, CNM, and Katelyn Tavares, APRN bring years of experience and are dedicated to providing the level of care our patients expect from SNHH.

Michelle Le, DO

Le_Michelle_10-3 - Copy.jpgBoard-certified Dr. Michelle Le is joining the team at Foundation Gastroenterology. Dr. Le earned her DO in 2015 from Edward Via College of Osteopathic Medicine in Blacksburg, VA. Dr. Le has completed an internal medicine internship and residency at Sinai Hospital of Baltimore. She has also completed a transplant hepatology fellowship at the University of Kentucky and an advanced interventional endoscopy fellowship at the University of Nebraska Medical Center.

Rachel Demers, CNM

Demers_Rachel_10-4 - Copy.jpgRachel Demers, CNM is joining our OB/GYN Hospitalist program. Rachel earned her Doctor of Nursing Practice-Nurse Midwifery in 2025 from Fairfield University in Fairfield, CT. Rachel also earned her Bachelor of Science in Nursing in 2019 from Fairfield University.

Katelyn Tavares, APRN

Katelyn earned her Master of Science in Nursing – Family Nurse Practitioner in 2025 from the University of New Hampshire in Durham, NH. She earned her Bachelor of Science in Nursing from the University of New Hampshire in 2015. Katelyn brings experience in a broad range of medical specialties.
