Pepperell Family Practice

The Pepperell Family Practice care team is committed to working together to support your healthcare needs while we actively recruit new providers. If your provider has left the practice or is retiring, the notification letter you received identifies the Primary Care Provider (PCP) who will assume your care. 

Pepperell Family Practice uses Epic, a secure electronic medical record system, to help keep your health information private and protected. Members of your care team have appropriate access to your medical record to coordinate and provide the care you need. 

As a patient of Southern New Hampshire Health, you have access to MyChart, a secure online portal that allows you to view portions of your medical record, communicate with your care team, request prescription refills, and manage appointments. You can access MyChart through this link. 

If you would like to obtain a copy of your medical records, please complete the appropriate Medical Records Release Authorization form:

Completed forms may be submitted according to the instructions provided on the authorization form. 

If you have an upcoming appointment with a provider who is no longer practicing at Pepperell Family Practice, a member of our team will contact you to reschedule your visit with another provider. If you have any questions about your upcoming appointment, please contact the office at (978) 433-6317.

If you would like to work with another provider in our healthcare system in New Hampshire, contact our HealthMatch team at 603-880-EASY (3279). Please note that Massachusetts patients with MassHealth and some Blue Cross Blue Shield of Massachusetts plans may not be eligible to receive care in New Hampshire. If you are unsure of access limitations related to your insurance, please call your insurance to verify.

When our office is closed, you can: 

• Contact an on-call provider by calling our office at 978-433-6317.  

• Visit one of our convenient Immediate Care locations or schedule an Immediate Care Virtual Visit 

For life-threatening emergencies, like chest pain or shortness of breath, call 911 or go to the nearest Emergency Department.

Medication management and continuity of care plans are a priority during this transition. Your new PCP or a member of the care team will review refill requests and follow up to either refill or schedule a visit to discuss medication planning. 

All pending tests can be completed as planned, and the results will be reviewed by your new Primary Care Provider (PCP), a member of the Pepperell Family Practice Care Team, or a Foundation Medical Partners (FMP) physician providing coverage for the practice. 

Referrals that were previously ordered remain active and can be used to obtain services. If an updated referral is required by your insurance company or additional visits need to be authorized, a provider at Pepperell Family Practice will be happy to assist.
