Each navigatorspecializes in a specific type of cancer, allowing for personalized guidance, education,and support tailored to each diagnosis while working closely with the care team tocoordinate comprehensive, patient-centered care.

At Southern New Hampshire Health, oncology nurse navigators can:

• Answer questions, provide education, and support informed decision-making ​

• Coordinate and schedule tests, treatments, and appointments ​

• Communicate with physicians and care teams to help streamline care and services ​

• Connect patients and families with supportive resources such as nutrition, physical therapy, social services, financial assistance, transportation, and community programs ​

• Provide emotional support throughout the cancer journey
